DISC benefits have varying waiting periods. This is meant to be an overview of employee benefits for regular full-time employees. DISC retains the right to amend or terminate any employee benefit at any time
Medical
Coverage is available to you and your dependents under the Preferred Provider Plan (PPO) through Care First, Blue Cross Blue Shield on the 31st day following your hire date. Vision discounts are also available under this plan. Your premium is fully paid by DISC. Dependent coverage is partially paid by DISC. deductions are made on a pretax basis. If covered elsewhere, you may decline medical coverage and receive cash or apply your flexible dollars to another benefit.

Dental
Coverage is available to you and your dependents through Guardian. Your premium is fully paid by DISC. Dependent coverage is partially paid by DISC. Coverage is available the 31st day following your hire date.

Spending Accounts
Medical and dependent care spending accounts can help you save money on your out-of-pocket expenses by paying for eligible services with pre-tax dollars.

Short Term Disability
After seven consecutive days of disability, if approved by the insurance carrier, you will be provided with 60 percent of your weekly salary up to a maximum of $625.00 per week. The premium is paid by DISC.

Long Term Disability
If your disability continues after a 12 week period, long term disability may apply. Benefits are 60 percent of your weekly salary up to a maximum of $10,000 per month. The premium is paid by DISC.

DISC provides 10 paid holidays per year

Paid Time Off
DISC provides from 5 to 8 hours off per pay period depending on length of service. There are 26 pay periods in a year.

401(k)
On the first of the month following employment, employees are eligible to participate in the company’s 401(k) plan. An employee may contribute from 2 percent of compensation up to the maximum allowed by law. The company matches employee contributions at 50 percent, up to a maximum company match of 4 percent of base compensation. Employer funds are vested over a period of six years.

Educational Assistance
The company provides up to $5,000.00 per year for approved undergraduate/ graduate courses with a grade of C or better.

Employee Assistance
The employee and his/her immediate family members are provided free counseling on personal matters.

Long Term Care
This is a voluntary program offered through UNUM to regular employees working at least 20 hours per week. DISC will provide the base long-term coverage at no cost to full-time employees that have annual base earnings of $45,000 or less.

Life
Employee paid at 1.5 times salary up to $50,000.

This is a voluntary program offered to regular employees working at least 20 hours per week.
